- Active California Advanced Practice Registered Nurse (CA APRN)
license
- A minimum of two (2) years of experience in primary care
(adult) as a Nurse Practitioner and within the last three
years.
- NP residents should be recent graduates, completing their
accredited Masters or DNP program no more than twenty (24) months
prior
- Must be credentialed or license/credential-eligible as a Family
Nurse Practitioner (FNP-C) or Adult Gerontology Nurse Practitioner
(AGNP-C)
- Specialty Services: In addition to the above requirements, NPs
who are requested to work in specialized areas such as Emergency
Medicine, Surgery, etc., must also have at least two years (2)
years of experience within the last 3 years providing patient care
within that setting and possess any required certificates.
- Obstetrics/Gynecology: NPs who will be staffed at female
institutions/facilities must possess a minimum of one (1) year of
experience within the last five (5) years providing obstetric and
gynecological nursing or patient care services in a healthcare
setting.
- Clinical competencies must be up-to-date and validated for the
specific area in which the NP will be assigned. Responsibilities:
Under the guidance of the Institution/Facility CEO/CME or their
designee, the NP will deliver services within the scope of practice
for NP services, adhering to Institution/Facility rules,
regulations, policies, and procedures.
- Standards of Obligation: Provide high-quality services,
consistent and in alignment with established and widely accepted
standards and principles of medical practice. This Agreement does
not override the common law rules governing the interpretation of
established and widely accepted standards and principles of medical
practice.
- Treatment Authorization: Except for emergency and primary care
services, NP must obtain prior written authorization from the
respective Institution/Facility CEO/CME or designee, as per the
Utilization Management Plan. All necessary forms related to the
treatment of patients/youth must be completed and submitted.
Authorization for essential services must be documented in the
patient/youth medical record before considering non-emergency
specialty treatment, consultations, or diagnostic procedures not
specified in a prior authorization form.
- Transportation/Transfer Authorization: NPs are not permitted to
transfer a patient/youth to any Institution/Facility without prior
written authorization from the appropriate Institution/Facility
CEO/CME or designee.
- Experimental and Investigational Drugs and Procedures: NPs are
prohibited from performing or administering any experimental or
investigational treatment, therapy, procedure, or drug to any
patient/youth unless authorized under law. Such treatment must have
prior authorization from the Institution/Facility CEO/CME or
designee and appropriate Office of Legal Affairs, unless otherwise
permissible by law. NPs are expected to perform or administer only
those medical services that are recognized as being in accordance
with generally accepted professional medical standards and deemed
safe and effective for treating the relevant illness, injury, or
condition, subject to the limitations outlined in this Agreement.
- Exclusions and Limitations: NP shall agree that no healthcare
service or treatment shall be provided for conditions specifically
listed in California Code of Regulations (CCR), Title 15, Division
3, Chapter 1, Subchapter 4, Article 8, Section 3350.1. However, NP
may seek individual case exceptions for excluded or limited
healthcare services or treatment by obtaining prior approval from
the Institution/Facility's CEO/CME or designee. NP shall agree that
any excluded healthcare service or treatment required for
pre-existing conditions will be provided only in cases where the
condition has become aggravated to a significant threat to the
patient's/youth's current health. Treatment is permissible if
failure to provide it would result in morbidity and/or mortality.
The appeal for such treatment must be recognized as medically
necessary. When prior authorization is granted, the Medical
Standards of Care shall be used as a reference.
- Clinical Reviews: Upon request from the Institution/Facility's
CEO/CME or designee, NP will conduct clinical reviews to assess the
quality of clinical care provided. This may involve reviewing index
cases (deaths, adverse outcomes, etc.) to evaluate clinical
quality. As part of the clinical review process, NP may be required
to participate in Professional Practice Committee activities and
could be called as a witness during such activities. The clinical
review process may include examining written health records,
engaging in discussions with Physicians, Physician Assistants,
and/or Nurse Practitioners, as well as Facility supervisors, and
conducting direct patient/youth examinations. NP conducting
clinical reviews must possess the necessary skills to prepare
written clinical review documents.
- A copy of the formulary will be provided and kept on file by
the NP. NP will consult with NP or Physician to ensure that,
whenever possible, only medications listed in the formulary are
prescribed, unless prior authorization is granted by the
Institution/Facility's CEO/CME or designee for any deviations.
